This report presents Minnesota’s Family First Prevention Services Act, Title IV-E Five-year Prevention Plan. It begins by explaining the child welfare vision for placement prevention in Minnesota, child welfare values, the plan’s objectives, and the Minnesota Department of Human Services’ ongoing efforts to build a continuum of care that effectively meets the needs of all children and families. Community services and support provided, government services and supports provided, voluntary child welfare system services and support provided, and child protection in-home services and supports are described, as well as collaboration and consultation with partners and with Minnesota Tribes.
